How do you recommend testing a product that can be shipped and returned numerous times during its lifecycle?

I have a client that ships medical devices out to a customer, who selects the ones they need from the container (due to size) and then returns the rest. Upon receipt back they replenish the contents and ship out to next customer. Issue is some low use items may get 100 does one best simulate that for vibration testing? I see articles on accelerated vibration testing but this may only get you to double the simulated miles per test within the 3 hours. Does one just run the test for days duration versus hours?
